spawn: improve wrapper support

Avoid type conflicts if overriding system header.

* m4/spawn_h.m4 (gl_SPAWN_H): Check for type existence.
(gl_SPAWN_H_DEFAULTS): New defaults.
* modules/spawn (Makefile.am): Substitute them.
* lib/spawn.in.h: (posix_spawnattr_t, posix_spawn_file_actions_t):
Only declare if missing or broken.
